Collect Digital Signatures in Google Forms

Using Google Forms and DocSketch, you can easily collect digital signatures from parents and students. Click Here for Video Instructions
1. Go to forms.new and create a new Google Form.

2. Add the 'File Upload' question type as one of your questions and name it Signature.

3. Add a description to the 'question' giving the user instructions to upload a digital signature they already have or to create a new signature using DocSketch.  https://www.docsketch.com/online-signature/draw/

4. Change your Form settings so that anyone can access the Form.

5. Get the shareable link to your Form and send it to the necessary users.

6. View your responses in a spreadsheet to see the Form entries and signatures.
